• USA Hockey Referee Application Form
  Online registration with USA Hockey is the preferred method of registering to become a certified official. However, if you are unable to register online, you can use this form. The form should be filled out and sent to USA Hockey, along with the registration fee applicable to the level at which you are registering.
  
  
• New York District Screening Form
  This form must be completed by any official who has reached his/her eighteenth (18th) birthday on the day of the officiating seminar which s/he attends. Only pages 7, 8, and 9 are to be filled out and submitted.
  
  
• New York State Match Penalty Incident Report Form
  This form, available in both MS Word and PDF formats below, should be completed and returned to the Local Administrative Supervisor of Officials within 48 hours of the game in which a Match Penalty was assessed. Please refer to the NYSAHA Rules page for more information.
